Archaeological forms are the basic document of record for archaeological sites throughout Louisiana, and are of vital importance to documenting our archaeological record. In general, archaeological sites are defined as a collection of at least five artifacts and/or a feature 50 or more years old.

 

The Division has two different types of forms: a Site Record Form and a Supplemental Form:

  • Site Record Forms should be used to record a newly discovered archaeological site or should have “Update” box checked if a site is revisited. The Site Update box should be checked when the recorder has actually stepped foot on the site, regardless of investigation methods.
  • Supplemental Forms should be used for adding additional information to the Site Files when no physical visit was made to the site. These additions include changes to the mapped site boundaries, landowner changes, recently received radiocarbon dates, analysis of a private collection, etc.

Process:

As of September 1, 2018: site numbers will be assigned upon finalization of the forms.

  • Upon completion of a field project, the draft site forms (for both newly reported and previously reported sites) are submitted to the Division. Please refer to the Site Form Instructions document below for specific instructions on submitting forms.
  • The Division will issue written comments within 10 business days, depending on the number of submissions and the quality of the forms. Batches of 20 or more site forms may take longer.
  • The state site numbers are required for the discussion of all sites within a project area in management summaries, draft reports and final reports.
  • Revised forms should be submitted within 30 days of receiving comments from our office. If the archaeologist/company does not return revised site forms to the Division by the specified time limit, the Division may not assign any new site numbers to that company.

The Division of Archaeology made modifications to the existing site forms, checklist, and instructions on October 4, 2021. If you are using the site form templates downloaded prior to this date, please replace with the new templates below. As of December 2021, all site forms (for new sites and site updates) and supplemental forms must be on the new form templates. We will not accept previous versions of the site form; however, revisions to forms submitted prior to the deadline will be accepted on the previous site form version.

 

The most recent site form includes the instructions and the checklist. When submitting a draft site form, delete the instructions and include the checklist. The checklist should not be included with final site forms. The instructions and checklist are also included below as separate files. If using an older version of MS Word, please use the compatibility form.
